Connected care saves lives

When a pediatric patient with a complex seizure disorder repeatedly turned to the Emergency Department (ED) for help, the underlying issue wasn’t the condition—it was the system. The child’s care was fragmented: an out-of-state specialist, multiple unconnected pharmacies and repeated delays in getting critical medications left the family struggling alone.

St. Luke's Health Plan became aware of this patient after an ED visit, where our team identified a series of life-threatening medication errors that had gone undetected. Leveraging our integrated model—where delivery system, physicians, pharmacy and insurance are aligned— St. Luke’s was able to: 

  • Detect and correct a dangerous dosing error 
  • Reconcile conflicting prescriptions across multiple pharmacies 
  • Coordinate directly with providers to secure the correct medication 
  • Provide the family with clear education and support to prevent future issues 

Because our teams and technology are connected, we closed gaps that the traditional, siloed approach missed—dramatically reducing the risk of another emergency visit.
